9D: Supporting Primary Care Reform through Multi-Payer Partnership

Earlier this month, CMS announced Comprehensive Primary Care Plus (CPC+), an advanced primary care medical home model and its largest investment in primary care transformation to date. CPC+ will bring together Medicare and other payers, including commercial insurance plans and state Medicaid agencies, to provide the necessary standards, financial support, and technical assistance for primary care practices to make fundamental changes in their care delivery. This session will highlight the critical role of multi-payer alignment in strengthening primary care as the foundation of our value-based health care system, as well as provide information for payers about partnering with CMS in CPC+.
